1. Question: Who is HealthMedia?
Answer: HealthMedia is a nationally recognized health improvement company, owned by Johnson & Johnson, that is among the most experienced in helping people live healthier lives. The company provides online tools and resources such as online health assessments and digital health coaching to help maintain and improve health.
2. Question: When I link to the health assessment, my browser says Medivonhealth. What is Medivonhealth?
Answer: Medivon is the company managing the Health Smarts portal and helping to administer the HealthMedia health assessment.
3. Question: I registered on the Health Smarts portal and then I was asked to register again on the HealthMedia site. Can I use the same user name and password on both sites?
Answer: Yes. You can use the same user name and password on both the Health Smarts portal and the HealthMedia website.
4. Question: When I went to take the health assessment, I got a notice that said I needed to enable my cookies. What is that referring to and how do I do that?
Answer: A "cookie" is a small piece of text stored on your computer by a web browser. Cookies contain bits of information such as user preferences, shopping cart contents and other data.
To participate in the health assessment and digital health coaching, you may need to enable the cookie feature on your computer. To enable cookies in Internet Explorer 8.0, you will follow these steps:
For more specialized cookie setting, click on "Advanced," check the "Override cookie handling" button and modify the settings to suit your requirements.
If you have an Apple or are using either another browser or an older version of Internet Explorer, you may need to research this question online.
5. Question: When can I take the health assessment?
Answer: You can take the health assessment at your convenience beginning January 4, 2010. If you take it prior to June 1, 2010, you will be eligible for a $40 cash award card. (Kaiser participants will receive a $50 award card.)
6. Question: Where do I take the health assessment?
Answer: To take the health assessment, click here and log in to get started.
7. Question: Can I take the health assessment before I go to the onsite health screening?
Answer: Yes. You can take the health assessment at your convenience, either before or after you attend the health screening event.
8. Question: What incentive do I receive if I take the health assessment?
Answer: If you take the health assessment before June 1, 2010, you will receive a $40 cash award card. (Kaiser award card is $50.)
9. Question: Why are there two versions of the health assessment?
Answer: HealthMedia has designed two different versions of the health assessment to appeal to the needs of the people who use their website. If you want a quick "snapshot" of your health, you can complete the "Succeed Snapshot Questionnaire." If you want a more complete picture of your health, you can complete the full "Succeed Questionnaire." No matter which one you choose, you will receive a $40 cash award card (Kaiser award card is $50) after you click "submit" to submit your completed questionnaire.
10. Question: Will I get the cash award card if I take the long version of the health assessment?
Answer: Yes. You will receive a cash award card after you complete either of the Succeed Questionnaires. It’s up to you whether you want a quick snapshot or a more complete picture.
11. Question: If my spouse participates in the health assessment do they get a separate HealthMedia account?
Answer: Yes. Every person who registers on the Health Smarts portal and HealthMedia website will have their own distinct accounts with separate user IDs and passwords
12. Question: What happens after I take the health assessment?
Answer: After you complete the Succeed questionnaire, your health assessment questionnaire will be processed and you will get a screen that says "Your personal guide is ready—click here."
Read through the plan on the page that indicates your health status and click "Next" to access your health focus area, also called "Your Snapshot Focus." Read through the pages that follow for important information on your health focus area.
You can immediately sign up for digital health coaching. Click the gray tab at the top of the screen that says "Coaching." Find the coaching program you’re interested in and click "Go."
13. Question: I took the health assessment last year. Do I need to take it again this year to get the award card?
Answer: Yes. You must take the health assessment this year to receive an award card. To be eligible, you need to take the health assessment by June 1, 2010.
14. Question: What triggers the payment of my cash award card?
Answer: You must complete the online health assessment questionnaire and click "Submit Questionnaire." It will be processed and you will get a screen that says –"Your personal guide is ready – click here." At this point, you will have met the requirement for completing the health assessment.
15. Question: I don’t want to receive anything in the mail from HealthMedia. Do I have to provide my mailing address on the personal profile?
Answer: It is important that you enter your contact information so your award card can be mailed to you. Your mailing address will not be used for any other purpose.
16. Question: I took the health assessment last year. Will my data be uploaded to my new HealthMedia account?
Answer: No. We are using a new vendor, HealthMedia, to administer the health assessment. The health assessment we are using this year is completely different than those we have used in past years.
17. Question: I know many of my health numbers. I’d like to include them in my health assessment. Which health assessment should I complete?
Answer: You are not required to enter your health numbers in the health assessment questionnaire. However, if you want to include your health numbers, you should complete the Succeed Questionnaire and not the Succeed Snapshot.
18. What is my "personal guide" that I receive after taking the health assessment? Is this my digital health coaching plan?
Answer: The "personal guide" you receive after completing your health assessment is NOT the digital health coaching plan. It is only a report on your health.
To get your digital health coaching plan you need to sign up for it. Click the gray tab at the top of the screen that says "Coaching." Find the coaching program you’re interested in and click "Go."
19. Question: While working on the health assessment, I left my computer for 15 minutes. When I came back I had a screen that said "your session has timed out—please enter your access code." What is an "access code?"
Answer: The access code is another term for your password. Enter your user name and password and the screen will return to where you left off.
20. Question: What do I do with my cash award card once I have used the balance on it?
Answer: If your award card only had $40 on it, hold onto it. If you engage in digital health coaching it will automatically be reloaded to include the cash award for digital health coaching.
21. Question: How do I print off my personal guide?
Answer: To print off your personal guide, click "Printer-friendly plan" under "My Succeed Plan" on the left navigation.
